The project
MedKick is an end-to-end telehealth platform. Care teams work from a desktop workspace where they see patients over video, manage their patient panels, and coordinate follow-up, while patients use a companion app on the other side of the relationship. Together they keep care going between office visits, not just during them.
Virtual visits
Video consultations are built into the workspace rather than bolted on. A clinician can start a visit, manage the microphone and camera, and end the call from the same screen where the patient’s care plan and next steps are already in view.
Patient panels and care plans
Every patient sits on a panel with their care plan, their next step, and a clear status: on track, due soon, in progress, or needs attention. Care plans cover chronic care, transitional care after a hospital stay, preventive care, and behavioral health. Clinicians can switch between all patients, their own panel, and the patients who need follow-up.
Care team coordination
A patient’s care rarely belongs to one person. MedKick brings the care manager, primary care, behavioral health, and pharmacy into one view with each member’s availability, alongside team messages and the care tasks due this week, so handoffs happen in the platform instead of in inboxes.
Remote monitoring between visits
Connected devices send readings between appointments, and nurse supported care coordination turns that information into action. Care programs and outreach keep patients engaged, and the dashboard tracks active patients, open care tasks, unread team messages, and outreach for the month.
Reporting and resources
Analytics and shared resources round out the workspace, giving practices a view of their care programs and giving teams the reference material they use with patients.
The patient app
The companion app is the patient’s side of MedKick: the place they join virtual visits and stay connected to the care team that is following their progress between appointments.
