Health Plus
A project centered on the administrative work around chronic care management.
The project
Health Plus was a chronic care management workflow project focused on the administrative work around patient support. The brief centered on call documentation, follow up planning, and care coordination.
What we delivered
The work produced a proposed workflow and a focused direction for demonstrating the experience. It brought documentation and follow up into one project narrative, explaining how information captured during a call could support the next coordination task.
How we approached it
We organized the work around three connected activities: documenting a conversation, planning the follow up, and coordinating the next action. This narrowed a broad care management challenge into a specific sequence that could be explained and reviewed.
How the pieces fit together
Call documentation provides the context. Follow up planning identifies what needs to happen next. Care coordination connects that next step with the people responsible for it. The project used this relationship to define the proposed experience rather than presenting unrelated features.
The completed scope
The available project materials document the proposal and workflow strategy. They do not establish a deployed clinical platform or live integrations. The deliverable presented here is the defined workflow and project direction.
